Step 1: Please complete all items on this form.
Step 2: Send an electronic (.SVG format) file version of your Agency/Company logo.
Step 3: Create a list of Credentials. These should be any item you will need to track on an individual Temp (clinician) for either Agency employment or Client placement. Think about categories as well as individual credentials. Four categories (CPR Levels, Advanced Certifications, Medical, and HR) and Credentials indicated under each category will be included on your site. During training, you will be instructed on where additional items are added (if applicable). You can add categories, in addition to credentials. Creating a list may help you during the training session for setup.

Administrative Users and Authorized Contacts
Administrative Users will have FULL RIGHTS and will set up additional users and permissions via the User Manager area in each Solution.
Provide a list of Authorized Contacts who may contact our Support Team. The Support Team will confirm
they are engaging with a designated Authorized Contact for security purposes.
